Real questions.
Real stakes.
Implant torque at placement — 35 Ncm or 45 Ncm in D2 bone? I keep getting conflicting guidance.
Three years out of residency. My mentor says 35, the manufacturer says 45, the last CE course I took said "it depends." I placed 14 implants last month and I'm still not confident I have a defensible protocol. Anyone willing to share what their actual number is and why? Attaching my last three case radiographs.
Delta Dental clawback on a crown seated 18 months ago — is this actually enforceable?
Got a letter demanding repayment of $1,240 for a crown they say doesn't meet "clinical necessity criteria." The crown is still in the patient's mouth, functioning perfectly. My documentation was thorough. Has anyone successfully fought one of these? I'm trying to figure out if this is a negotiating tactic or a legitimate audit.
Evidence base for 3-month vs. 4-month perio maintenance intervals — what does the 2025 literature actually say?
Preparing for Monday morning huddle. Our current protocol is blanket 3-month recalls for all perio maintenance patients. I found two 2024 RCTs suggesting 4-month intervals are non-inferior in Stage II patients. Trying to build a case for individualized scheduling. Looking for anyone who's made this transition — what resistance did you get from the practice owner?

Editorial note
These aren’t cherry-picked. They are the three most-viewed threads opened in the last 72 hours. The questions are uncomfortable because they should be — these are the gaps between what dental school teaches and what Tuesday at 3 PM actually asks of you.

Six categories.
Every question fits.
Clinical Cases
The core of the forum. Radiographs, photos, differential diagnoses, treatment planning debates. Where the most urgent questions live.
Hottest current debate
“At what point is a tooth with 40% bone loss truly unrestorable vs. strategic extraction?”
Implantology
Placement, restoration, complications, torque protocols, bone grafting, and the never-ending tissue management debates.
Hottest current debate
“Immediate loading protocols in the esthetic zone — who's doing it and under what conditions?”
Practice Management
Insurance reimbursement, associate contracts, overhead benchmarks, billing disputes, and the business questions dental school didn't cover.
Hottest current debate
“DSO vs. independent ownership at year five — a brutally honest thread”
Material Science
Composite vs. ceramic comparisons, bonding protocols, material failure analysis, and early reads on new products before the rep gets to you.
Hottest current debate
“Zirconia monolithic crowns at 3 years — the wear data is in, and it's complicated”
Hygiene & Periodontics
Protocol updates, recall interval evidence, perio-systemic links, and the hygienist-dentist communication threads that get surprisingly deep.
Hottest current debate
“Evidence for laser-assisted perio vs. traditional SRP — separating marketing from RCTs”
Career & Wellbeing
The thread category no one mentions at conferences. Burnout, second opinions on career pivots, associate negotiation strategy, and the questions you ask anonymously.
Hottest current debate
“Burnout at year three: is it the specialty, the practice model, or something else?”
Index note
The taxonomy emerged from member behavior, not editorial planning. Categories with fewer threads are not less important — they are newer. Career & Wellbeing grew 140% in 2025 after an anonymous post about clinical depression in solo practice received 94 replies in 48 hours.
Total forum corpus
I found my answer in a four-year-old thread at 11 PM the night before a complicated extraction. Someone had documented the exact same anatomy. That thread might have changed the outcome.

DentalCommons is not a CE platform. It’s not a directory. It’s not sponsored by anyone. It’s a place where the profession talks to itself — honestly, specifically, and without the usual filters.
The best threads here will never appear in a journal. That’s the point.
